Pelvic venous disorders and orthostatic intolerance: A systematic review of diagnostic associations and treatment outcomes

Possible pathophysiological relationship between PeVD and OI. Nutcracker syndrome occurs when the left renal vein is compressed between the superior mesenteric artery and the abdominal aorta, impeding renal and systemic venous outflow. May-Thurner syndrome results from compression of the left common iliac vein by the overlying right common iliac artery, reducing venous drainage from the pelvis and lower extremities. Venous insufficiency reflects valve damage and venous laxity in peripheral veins, leading to abnormal retrograde flow and pooling. Collectively, these conditions promote infra-diaphragmatic venous pooling, diminished preload, and compensatory sympathetic activation, which can trigger orthostatic intolerance (OI) phenotypes such as postural tachycardia syndrome (POTS), orthostatic hypotension (OH), or dizziness and fatigue.
